September is Urology Awareness Month, a time dedicated to increasing awareness about urological health issues. One of the most common concerns among men, particularly as they age, is their prostate health.

The prostate gland, which plays a crucial role in male reproductive health, can be affected by several conditions, with Benign Prostatic Hyperplasia (BPH) being one of the most prevalent. Understanding the signs of an enlarged prostate and its potential implications is essential for maintaining good urological health.

What is Benign Prostatic Hyperplasia (BPH)?

BPH is a non-cancerous enlargement of the prostate gland, typically occurring in men over the age of 50. As men age, the prostate continues to grow, sometimes leading to BPH. This condition is extremely common, with studies indicating that over 50% of men in their 60s and up to 90% of men in their 70s and 80s experience some degree of prostate enlargement.

Why do men develop an enlarged prostate?

The exact cause of BPH is not fully understood, but it is believed to be linked to hormonal changes as men age. Specifically, the balance between testosterone and oestrogen shifts, leading to increased activity in the prostate gland. Another factor could be the accumulation of dihydrotestosterone (DHT), a hormone that stimulates prostate growth. Genetics, lifestyle, and other health conditions, such as obesity and Type 2 diabetes, can also play a role in the development of BPH.

Recent research has highlighted a potential link between BPH and Type 2 diabetes. A study published in BMC Urology found that men with Type 2 diabetes are more likely to develop BPH due to the insulin resistance associated with diabetes, which may contribute to the abnormal growth of prostate cells. Understanding this link emphasises the importance of managing overall health to potentially reduce the risk of developing BPH.

Recognising the symptoms of an enlarged prostate

The symptoms of an enlarged prostate are primarily related to the pressure the enlarged gland places on the urethra, the tube that carries urine from the bladder out of the body. Common symptoms of BPH include:

  • Frequent urination – particularly at night (nocturia), which can disrupt sleep.
  • Urgent need to urinate – a sudden, uncontrollable urge to urinate.
  • Difficulty starting urination – straining or needing to push to begin urinating.
  • Weak urine stream – a decrease in the force of the urine stream.
  • Incomplete bladder emptying – feeling that the bladder is not fully emptied after urination.
  • Dribbling at the end of urination – leakage of urine after finishing urination.

While these symptoms can significantly impact a man’s quality of life, it’s important to note that BPH is not cancerous. However, the symptoms of BPH can be similar to those of prostate cancer, making it crucial to differentiate between the two conditions.

Prostate Cancer and PSA testing

Prostate cancer is another common condition that affects the prostate gland, particularly in older men. While BPH and prostate cancer are distinct conditions, they can present with similar symptoms. Therefore, it is vital for men experiencing urinary symptoms to undergo a thorough urological assessment, which may include a Prostate-Specific Antigen (PSA) test.

The PSA test measures the level of PSA, a protein produced by the prostate gland, in the blood. Elevated PSA levels can be an indicator of prostate cancer, although BPH and other conditions can also cause increased PSA levels. As part of a comprehensive urology assessment, a PSA test can help in the early detection of prostate cancer, improving the chances of successful treatment.

Interestingly, recent research has suggested that BPH may actually serve as a protective factor against prostate cancer. A study published in The Prostate found that men with BPH might have a lower risk of developing prostate cancer. This counterintuitive finding highlights the complexity of prostate health and the importance of ongoing research in this area.

Sexual Health Week takes place from 9th to 15th September, and aims to highlight the importance of treating sexual health as we would any aspect of our health.

Women going through menopause have different experiences, but it is not uncommon for there to be sexual health concerns. The changes in menopause that can cause various responses, including low desire, vaginal dryness and pain during or after intercourse, can be extremely uncomfortable and distressing to manage.

Here, we discuss sexual health in menopause. We’ll also share how our compassionate team of female doctors can help women during this difficult phase.

Common sexual health problems in menopause

Although many women experience difficulties with sexual health during menopause, this does not affect everyone. There are many different factors that can contribute including medical history, psychological and social-cultural factors.

An Australian study involving over 2,000 women found that low desire, sexually related personal distress, and hypoactive sexual desire dysfunction (HSDD) are common in women at midlife.

Some possible changes in menopause that may impact on your sex life include:

  • Decreased oestrogen levels cause the vaginal tissue to become drier and thinner
  • Hormonal changes may affect your libido
  • Night sweats can cause disturbed sleep, making you tired
  • Emotional changes can impact on mood, making you stressed or irritable

With the right support, many of these issues can be treated with safe and effective therapies.

Sexually transmitted infections in menopause

Although menopause can cause different challenges for older women with their sexual health, there is still the same risk of catching an STI.

A recent Australian study showed that STI rates were increasing faster in older women aged 55 to 64 years compared with younger age groups.

This highlights the increased risk of STIs for women in menopause. Biological changes such as decreased oestrogen levels and vaginal atrophy can lead to thinner, more fragile vaginal tissues.

These changes make the vaginal environment more susceptible to infections.

For women who are concerned about any symptoms of an STI, EssexPrivateGP offers a sexual health screening package. Our experienced doctors can recommend the right tests, that can be performed in-clinic or at one of our recommended diagnostics clinics, and will provide a quick diagnosis, tailored treatments and advice.

Treatments for women going through menopause

At EssexPrivateGP, we have a team of experienced female doctors who offer comprehensive menopause care, focusing on personalised treatments that address both general symptoms and sexual health concerns.

Our personalised care plans include lifestyle advice and support for managing mood changes, sleep disturbances, and other menopausal symptoms. For example, we will help identify certain triggers of symptoms, such as alcohol or caffeine, so these can be excluded.

For changes during menopause affecting sexual health, we can provide Hormone Replacement Therapy (HRT) to alleviate symptoms like vaginal dryness and low libido. For those who cannot, or prefer not to have HRT, we have non-hormonal options including plant phyto-oestrogens such as red clover, sage and sea buckthorn oil.

Get in touch to book a menopause consultation today by calling 01277 568562 or email contact@essexprivategp.com and discover personalised treatments to enhance your overall well-being and sexual health during menopause.

Menopause is a normal ageing event that usually occurs between 45 and 55, with 51 deemed the average age for women in the UK. In addition to this physical transition, this time is often full of other transitions. Women may be caring for ageing parents, supporting children as they move into adulthood, starting new relationships, or taking on new responsibilities at work.

Despite being a normal biological process, the physical and emotional changes experienced during menopause vary widely. Some women barely notice these changes and may even feel relieved they no longer need to worry about painful periods or getting pregnant.

For others, the menopausal transition can have a significant and negative impact on their quality of life.

Common symptoms of the menopause

  • Hot flashes
  • Sleep issues, including insomnia
  • Night sweats
  • Increased heart rate
  • Mood swings, such as irritability, depression, or anxiety
  • Vaginal dryness and discomfort during sex
  • Urinary issues, like incontinence or frequent urination
  • Reduced libido

The stages of the menopause

Menopause consists of three main stages: perimenopause, menopause, and post-menopause. During these phases, the ovaries shrink, and the levels of hormones that regulate menstruation, such as oestrogen and progesterone, decrease. Additionally, the number of eggs in the ovaries naturally diminishes with age, reducing fertility. Transitioning through these stages typically takes seven years or more.

Knowing the stages of menopause can help you manage symptoms and recognise when to consult your GP for advice on your options.

Perimenopause or pre-menopause

Perimenopause, also known as pre-menopause, occurs three to five years before menopause. The age at which symptoms start varies, with most experiencing them in their mid to late forties, though it is rare before age 40.

In this stage, hormone levels begin to fluctuate, leading to mood changes, irregular menstrual cycles, and other symptoms. It is still possible to conceive, so using birth control is advisable.

Menopause

Menopause is defined by the absence of menstrual periods for 12 consecutive months, not caused by illness, medication, or pregnancy. At this point, pregnancy is no longer possible.

The average onset age is 51, but it can occur from the mid-forties to mid-fifties.

Post-menopause

Post-menopause marks the end of reproductive years, with ovaries producing minimal hormones and menstruation ceasing.

This stage lasts for life, with some symptoms persisting for years after the last period but generally decreasing over time. However, the risk of heart disease and osteoporosis increases due to lower oestrogen levels.

Fortunately, menopausal symptoms can often be managed through a healthy diet, regular exercise, and lifestyle changes. If symptoms severely impact life quality, consulting your GP is advisable. Treatment can include hormone replacement therapy (HRT) or other medications to manage symptoms, enhance wellbeing and help you view this as a positive transition into a new life stage.

In honour of Love Your Parks week, we’re focusing on the benefits of parkrun and why the Royal College of General Practitioners (RCGP) advocates its use as a ‘prescription’ to enhance health and wellbeing, potentially reducing the need for long-term medication.

Parkrun began as a modest gathering in Bushy Park, London, in 2004, where 13 friends participated in a timed 5km run. Today, it has evolved into a global event with over 350,000 participants worldwide every Saturday. Parkrun has become a staple activity for runners of all abilities.

In 2018, the RCGP collaborated with parkrun, encouraging GPs to promote it to their patients. Since then, research has highlighted the positive impact on individuals with various chronic conditions, including diabetes and depression.

Three benefits of parkrun

1. Increases life expectancy

A new study published in the British Journal of Sports Medicine analysed health data from over 30 million people in the UK and found that walking for at least 11 minutes daily could reduce the risk of premature death by nearly 25%.

2. Reduces risk of serious health concerns

Research published in the British Journal of General Practice indicated that over 9% of participants and 45% of walkers had at least one long-term health condition, such as arthritis, obesity, hypertension, chronic pain, anxiety, depression, or Type 2 diabetes. Regardless of their finish time, participants reported improved physical health.

3. Improves mental health

A 2018 study by Glasgow Caledonian University revealed that 89% of parkrunners felt happier, had improved mental health, and enhanced body image through participation.

In fact, you don’t even have to run to feel the benefits. A Sheffield Hallam University’s Advanced Wellbeing Research Centre (AWRC) survey involving over 60,000 parkrunners found that volunteering at parkrun also provided benefits. Eighty-four per cent of volunteers reported feeling happier due to their involvement.

This community connection, an essential aspect of parkrun, contributes to its success.

If you wish to join in and experience the benefits of parkrun, the Billericay parkrun takes place every Saturday at Lake Meadows Park at 9 am. Or for more advice on making sustainable lifestyle changes to improve health and reduce disease risk, call 01277 568562 to make an appointment with one of our GPs.

I would like to take this opportunity to extend my deepest thanks and give the highest possible recommendation to Dr Eka, who has been absolutely instrumental in my daughter’s ongoing care following a particularly aggressive and resistant UTI infection. Had it not been for his exceptional dedication, knowledge and out of hours support, I dread to think what the outcome could have been. My daughter had previously been given advice and medication by our regular GP that unfortunately was not sensitive to the bacteria that had grown, not once but twice. Each time I was reassured that the infection had cleared as her symptoms appeared to settle. However, Dr Eka showed outstanding diligence and care. He advised a different course of medication purely as a precaution, advice which, had I not followed, could have resulted in my daughter becoming seriously unwell. Despite it being out of hours, he offered round the clock guidance, reassurance and support. He even said he would carry out a home visit should her condition worsen. After navigating an incredibly stretched NHS system, this level of care was profoundly reassuring. I cannot recommend Dr Eka highly enough. His kindness, professionalism and genuine commitment to patient care truly go above and beyond. I will always seek his advice in the future should we ever need it again, although I sincerely hope we do not.

Dr. Lach is nothing short of an outstanding physician! I first saw her some time ago for a personal matter, and the experience was exceptional. Unlike many of my past experiences elsewhere, Dr. Lach took the time to complete a thorough case history and an extensive physical examination. At a time when so many doctors are under immense pressure and limited for time, she stands out by ensuring her patients receive the care and attention needed to truly find the root cause of the problem. As the Director of The Chiropractic Centre – Billericay, I place great value on professionalism, respect, and attention to detail. Dr. Lach embodies all of these qualities—she is open-minded, focused, and deeply committed to her patients. Because of this, she has become my number one GP of choice, and I continue to refer a number of my own patients to her. If you want exceptional medical care, come here. Doctors like Dr. Lach are worth their weight in gold!

Dr Akan Eka has been ever so incredibly support everytime i have been to him he has been very thorough with any health conditions i have raised. The reason why i felt the need to write a review as i don't normally, is because he saved my life. If it wasn't for Dr Eka being so knowledgeable after being failed by other doctors and paramedics i may of not been able to talk again. I reached Dr Ekas out of hours contact number as i was having some trouble after having a reaction to medication, this was ignored by 4 paramedics and made me believe it wasn't anything to worry about although i couldn't speak "it's just anxiety" meanwhile my tongue was hanging out my mouth unable to talk. I ended up believing it's just me and anxiety is that bad that i'm doing this to myself. I message Dr Eka and he knew exactly what had happened within minutes of exchanged messages. This condition needed to be blue lighted to hospital and i was neglected by the paramedics. If i hadn't had Dr Ekas out of number i would not of gone to hospital and i potentially wouldn't have been able to talk again as this was all related to my brain. I can't thankyou enough.

This was my first visit to the Essex Private GP in Billericay having tried to call my own GP several times with no luck. I booked an appointment on their website, same day, paid the fee and turned up for my appointment. Within minutes of arriving, I was seen to by Dr Lach who was lovely and took her time to understand my symptoms and to do various checks. I spent longer in conversation with Dr Lach than I did making the appointment and in the waiting room. My experience of my local GP is the complete opposite. Yes admittedly it's a premium to pay to see a doctor, but after 48 hours I am feeling loads better and my experience of making and going to a GP appointment was seamless and actually really enjoyable. Had I kept trying to get an appointment with my local GP, it could have taken days and I would have got more sick, been more stressed and this would have impacted my work even more. I strongly feel that when your health is concerned, you get what you pay for and I honestly wouldn't think twice to go privately again.
